Please fill out this form and then print two copies of it.  One is for your records and the other must be sent with the cell phone or PDA.  After you have done that, click the submit button at the bottom of the page and a copy will be sent to us.  You will receive a confirmation from us indicating that we are aware your phone or PDA is coming to us for repair. 

There is a $15 charge for phones that are mailed in.  If you choose to have us repair your phone $10 of that will be credited against your repair charges. This covers our initial evaluation and the return shipping fees.  Your confirmation email will include payment information.  No device will be evaluated or shipped back to you until those fees are paid.  Once we determine what is wrong with your phone and the cost to repair it we will notify you.  Devices left over 30 days without payment become the property of Cell Phone Repair of Atlanta.
